Meet the West Health Accelerator Team: Angela Figueiredo, PT, DPT, CCM

Angela Figueiredo is a post-acute case manager at Brigham and Women's Faulkner Hospital.

She was eager to participate in the West Health Champions Program because she felt it would offer her a unique opportunity to enhance geriatric care through best practices, training programs and innovative care models.   

Q&A

Why are you passionate about caring for older adults?

Angela Figueiredo: I'm passionate about caring for older adults because their needs are complex, and they deserve both compassionate and thoughtful care. Throughout my career as a case manager and a physical therapist, I've seen older adults fall through the cracks, and I've also seen how powerful it can be to have the right supports in place when they leave the hospital. It's incredibly meaningful to help people age with dignity, maintain their independence, and make their own care decisions.

What’s one small change that can make a big difference in the care of older adults?

AF: One small change that can make a big difference is community support. I've seen many patients discharged home from the hospital without the right supports in place to maintain their health at home. I think it's really important for us to ensure older adults have the supports in place to help them feel connected, supported and cared for once they are home.

What do you hope to achieve in the West Health Champions program? 

AF: As an expert in transitions of care, I am committed to improving care coordination and ensuring seamless transitions for older adults. This program will provide me with valuable insights that I can apply to designing more effective transition plans and strengthening coordination within my field. Additionally, I look forward to sharing this knowledge with our Mass General Brigham skilled nursing facility collaborative partners to elevate the quality of care across the continuum and improve patient outcomes at every stage.
